Referral Pathways Meeting will be held FRIDAY, SEPTEMBER 24 at 10am at Mount Sinai

Due to scheduling issues the Referral Pathways Meeting will be held FRIDAY, SEPTEMBER 24 at 10am at Mount Sinai (instead of Aug 26).

Hello everyone –

We will be meeting at 10am on Thursday, August 26, at Mount Sinai. Please let me know if you or someone from your organization can attend. It would be great to get as much input as possible regarding linking people from the community into care – or from care back into social services in the community.

We will discuss some of the lessons learned from Mount Sinai’s pilot testing project with Frost’d and other issues below. Please let me know if you have anything you would find helpful to discuss in a subgroup meeting.

* HCV Services Referral Pathway Map Committee Update

Next meeting:

o Getting patients from testing into medical care is significant challenge due to:
+ Clients lost to follow up: MIA (no phone, no permanent address, unresponsive)
# Possible Solution: develop traditional and non-traditional follow up protocol for HCV clients – Oscar Lopez, Harlem United
+ Patients sometimes lack of sense of urgency to deal with HCV amongst other concerns
# Possible Solution: “HCV Coaching Program” or “HCV Care and Treatment Readiness Empowerment Program” Project – Guedy Arniella, Institutue for Family Health
+ Stronger linkages are needed to programs where people with HCV are getting services
# Possible Solution: Outreach, education and support to methadone programs and homeless shelters
+ Geography (distance from test site to medical care site)
+ Culture (difference in culture of test site and medical care site may make client feel uncomfortable/hesitant)
+ Certain health insurance plans not being accepted by certain institutions
# Possible Solution for all 3 above: Further Develop Referral Pathway Map to include all information needed to allow test sites to make optimal referrals
